can print and complete. After completing the forms bring them to the
VLPOA Management Office located at 10874 W. Chino Drive.
You will need the Adobe Reader installed on your computer to view and print these forms.

Facilities-Landscape And Common Grounds Maintenance Request Form:

Form. Anytime you find that a Ventana Lakes facility or Common Grounds requires
maintenance please fill out one of these forms and bring it to the Management
Office.

Architectural
Request Forms:
The following Architectural Request
Forms are to be completed and brought to the Management Office for review by the
Architectural Control Committee. Approval by the Architectural Control
Committee is required prior to any homeowner making any alterations to the
exterior of their homes per the CC&Rs, Article IV, Section II.

Surepay Electronic Deposit Form:
Surepay Electronic payment for your Association assessments is available and
recommended. The Surepay form can be obtained here by clicking on the following link.
After you print and fill-out the form bring it to the Management Office with a
voided check to activate your Surepay Electronic Payment.

Gardens - Skylight Or Solar Energy Device Waiver And Liability
Agreement

Form for the Gardens. Because the Association is responsible
for the roofs of homes in the Gardens, if a property owner wants to
install a skylight or solar energy device on the roof, the following
form must be completed and delivered to the Association Office.

Emergency Medical Information Form:
The City of Peoria has provided an Emergency Medical Information Form to be completed by each occupant of a home. Download,
print, and complete this form, and then attach it to the back of your bathroom medicine cabinet. In the event of an Medical Emergency at your
home, the City Of Peoria Fire Department will check the back of your medicine cabinets for this information. In the event that you are in your home alone and become incapable of talking to the Paramedics, this information could be invaluable at providing life saving information.

Home Security Alarm:
The City Of Peoria requires that all residential security alarms be registered with the City.
Print the form, complete it, and mail it to the City Of Peoria
Police Department Alarm Registration Office. The contact person for the City of Peoria for Alarm
